Below you will find two documents that are required to be completed prior to volunteering during Friend’s sponsored events and MLSP park events in which you are participating as a volunteer.
The documents are valid for one calendar year and once completed will be securely stored in the park manager’s office.
These may be filled at at the first event you are volunteering for in the year, or you may fill them out and mail to:
